How would I create a character map like newchars.png?

Hi. The Cube 1 mod I'm making is coming along nicely, but I was wondering how to make a character map like newchars.png.

When I made my font, I just manually positioned new letters over the original in Photoshop, paintbucket-ed the background layer & merged the new layers down, and then just went through bugged it out. Expect some very tedious pixel adjusting, but the end result can be a lot of fun.
